Individuals who have Exercise Physiology cover on their health insurance can book Exercise Physiology sessions any time.
The item numbers available to be claimed are: 102, 202 and 502
You will have an initial session that is slightly longer to really get to the bottom of what you need.
During this session we will have an informal chat about your experience, how you are feeling, what you want out of your sessions and your past medical history. We may give you a few physical tests (if appropriate) to determine what you can do safely and provide us with a base line so we know when you have improved.
These tests may include Blood pressure, resting Heart rate and specific outcome measures relating to your individual condition and goals.
The Exercises you are given will be based on the results or the outcome measures your individual variables and your goals. Home exercises are often recommended to help you progress between sessions. On discharge we will be aiming to integrate you into a long term solution that is right for you so that you can make ongoing positive changes to keep you on track to a happier healthier life.
In addition to specific referral documents/case numbers/claims individuals should bring:
Individuals who have a chronic (long term) medical condition or who have a lasting injury effecting their health may be entitled to a Team Care Plan referral from your GP
Referrals are valid for 24 months, clients are eligible for a new referral of up to 5 sessions each new calendar year.
Doctors can either fax the referral through to us by Fax 03 8672 7703 or you can bring the referral with you
You will need to bring your Medicare card and a card that has a “savings” or “cheque” account available as Medicare does not rebate onto a “credit” account.
Individuals with an approved NDIS plan (studio and home visits available).
Plans can be through intermediary bodies (case managers) or individually managed by the client.
There are 3 types of funding that can be used to fund Exercise Physiology services:
Individuals with a Gold card require a GP referral letter and can have 10 sessions for all conditions.
Individuals with a White card require a GP referral every 10 sessions and can only be treated for the specific conditions listed on the GPs referral. The referral can be updated and modified if needed.
